NS5A resistance testing.

Appropriate for the following patients who have relapsed after at least 8 weeks of an HCV regimen containing an NS5A inhibitor, including the following:

1. HCV genotype 1 (subtypes 1a and 1b) patients who have relapsed after either VIEKIRA PAK or HARVONI

2. HCV any genotype who have relapsed after DACLINZA (daclatasvir) or EPCLUSA (sofosbuvir plus velpatasvir)

Information required (essential) on request form:

1. Genotype and subtype (assay is subtype dependent).

2. Viral load (should be available from 12 week post-treatment test).

Diagnostic Use and Interpretation

Hepatitis C virus is a ss RNA virus belonging to the family Flaviviridae.

Following infection, there is a highly variable incubation phase (2-52 weeks. The onset of disease is usually insidious, however, fluctuating ALT levels throughout the course of disease is a typical characteristic. One of the major problems associated with HCV infection is the high rate of asymptomatic infection and of carriage rates (both in the order of 70-80%). Possible outcomes of chronic infection include CAH, cirrhosis and the risk of liver cancer. Fulminant hepatic failure due to acute HCV infection is very rare.

Transmission is predominantly by the parenteral route. Sexual transmission does occur, however, it is an inefficient mode of viral transmission. The risk of perinatal transmission remains unclear.

Presence of anti-HCV in patients serum may indicate (a) a residual marker of previous resolved infection or (b) current / on-going infection. Following acute infection, antibody may take 2 ? 6 months to develop.
