Aspergillus Precipitins

Investigation of Allergic Bronchopulmonary Aspergillosis requires both Aspergillus Specific Ige and Aspergillus precipitin assays. These assays can now be requested together.

This assay has changed from an Ouchterlony double diffusion method to a FEIA method June 6 2017.

This will result in a more rapid TAT and more sensitive assay.

Levels above 40mgA/L are considered associated with chronic or allergic aspergillosis.

There is , however, considerable variation in IgG levels between individuals and patients with allergic bronchopulmonary aspergillosis may have values below 40mgA/L. Aspergillus specific IgG results must be interpreted alongside clinical features and, where indicated, serum IgE and Aspergillus specific IgE results.

Comparing local ImmunoCAP Aspergillus specific IgG results with clinical information suggests that the sensitivity/specificity of 40mgA/L for aspergillosis are 86%/43% and 70mgA/L as 56%/65%. We have therefore chosen the level of 40mgA/L to report against.

The sensitivity of 40mgA/L for >=3 precipitin bands is 92%.

Note: Minimum 200uL serum required for Aspergillus precipitin test.


Turnaround Time: At least 2 weeks

Tests are set up each Thursday and results are available the following Thursday.

Uncertainty of Measurement : 20%


Diagnostic Use and Interpretation

Aspergillus precipitins (which are IgG) are found in the sera of many patients with allergic bronchopulmonary aspergillosis (ABPA), as well as in patients with invasive aspergillosis.
