Important Information:

Testing is perfomed Mon- Fri only
Request form must indicate if the sample is pre -blood transfusion.
Samples received after 3pm on a Friday (or on the day before a stat holiday) will not be tested, unless they are pre-blood transfusion samples and prior arrangements have been made with the laboratory. A recollect will be required.

Specimen Collection
If sample is collected from within the Auckland region:

4 mL EDTA Blood
0.5 mL Paediatric Micro-EDTA Blood
Samples sent to LabPlus from outside the Auckland region:

Adult: 4 mL whole blood collected into a non-gel Heparin tube.

Paediatric: Microcollect 0.5 mL non-gel Heparin tube.

Transport at 4 degrees.

Sample age limit: Testing must be performed within 48 hours of collection.

Turnaround Time:

Preliminary verbal result to a referring Pathologist may be available within 36 hours of receipt of specimen, Mon - Fri.
The final report is available within 5 working days.

Diagnostic Use and Interpretation

Detection of a PNH clone.
Patients with paroxysmal Nocturnal Haemoglobinuria have an acquired somatic mutation of the PIG-A gene, located on the X chromosome. This defect results in reduced or absent expression of GPI linked proteins on the surface of red blood cells, granulocytic and monocytic cells. PNH clones have also been described in Aplastic Anaemia.
Assay involves detection of the following by flow cytometry: CD59 expression on red cells, Flaer and CD66b on granulocytes, Flaer and CD14 on monocytes.
Bone marrow samples are not recommended for testing for PNH clones - GPI anchor proteins show variable expression during myeloid maturation.
