Diagnostic Use and Interpretation

Cystatin C may be useful in the diagnosis and monitoring of renal disease.

Cystatin C is a small protein produced by all nucleated cells. The production rate is constant and is not significantly affected by gender, age and muscle mass.  Cystatin C is almost freely filtered by the normal kidney, and nearly completely degraded by the proximal tubular cells.  The plasma concentration of cystatin C is largely determined by the glomerular filtration rate (GFR). Non-GFR determinants include body fat percentage, thyroid diseases and use of corticosteroids.

Cystatin C may be superior to creatinine as a marker for GFR in some patient groups e.g. those with GFR  between 60-79ml/min/1.73m2, children and people with abnormally low or high muscle mass.
